This Privacy Policy describes how Willetts Technology collects, uses, and protects information when you use PUPS Check In by Willetts Technology. PUPS Check In is part of the PUPS Software family of products provided by Willetts Technology.

PUPS Check In is used by provider organizations and users to send wellness check-ins, satisfaction surveys, reminders, notifications, and account-related messages.

2. Information We Collect

Provider organizations may enter client or customer contact information and related service data in PUPS Check In. Depending on how the service is used, we may collect:

1. Names, email addresses, and phone numbers.
2. Check-in responses and survey responses.
3. Account data such as user names, roles, and organization information.
4. Usage data such as log data, device/browser information, and service activity.

3. How We Use Information

We use collected information to:

1. Operate, maintain, and improve PUPS Check In.
2. Deliver email and SMS notifications and service messages.
3. Provide customer and technical support.
4. Maintain system and account security.
5. Meet legal, regulatory, and compliance obligations.

4. HIPAA and Business Associate Terms

PUPS Check In may handle protected health information in connection with services provided to covered entities. Where applicable, such handling is governed by the Business Associate Agreement (BAA) between the provider organization and Willetts Technology.

5. SMS Privacy Statement

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties, except as necessary with service providers who assist in delivering the requested messages (such as SMS delivery providers).

SMS messaging is an optional delivery channel. Consent to receive SMS messages is not a condition of purchase, use of services, or receipt of care from a provider organization. Clients may opt out at any time by replying STOP to any SMS message.

6. Sharing and Service Providers

We may share information with service providers that help us operate the platform, including hosting, communications, analytics, and support vendors, only as needed to provide and secure the service or comply with law.

8. Your Responsibilities and Choices

Provider organizations are responsible for obtaining required permissions and consents, as required by law or policy, before entering client/customer contact information and before enabling SMS or email check-ins. SMS consent is optional and is not a condition of purchase or use of services.
